As early as possible - ideally as a puppy or kitten. Young, healthy pets have no pre-existing conditions, qualify for the lowest premiums, and will accumulate years of coverage before age-related conditions develop. Getting insurance after a diagnosis means that condition is permanently excluded.
Varies by provider. Some policies (Trupanion, Healthy Paws, Embrace) cover hereditary conditions like hip dysplasia and cardiac disease as long as they were not pre-existing. Others exclude hereditary conditions entirely. This is one of the most important questions to ask.
Most US pet insurance policies are reimbursement-based: you pay the vet, then claim from the insurer. You can use any licensed veterinarian, specialist, or emergency clinic.
